Yet the process is straightforward, automatic in some cantons, and can amount to several thousand francs per year. This guide explains who qualifies and how to go about it.

- The Confederation pays an annual contribution to the cantons (federal subsidy).
- The canton then decides who receives the reduction and how much.
As a result, the same income may entitle you to a reduction in one canton and nothing in another.
Who is eligible
Recipients are generally:
- Single-parent families or those with several dependent children.
- Young adults (aged 18 to 25) in education or training.
- People with modest incomes (employees, self-employed, retirees).
- Recipients of social assistance or supplementary AHV/IV benefits.
- Students without independent income.
Some benchmarks (to check in your canton):
- Family with 2 children: net income up to roughly CHF 80,000–100,000 per year depending on the canton generally opens a partial entitlement.
- Single person: net income up to roughly CHF 40,000–55,000 per year.
- Student: almost always eligible, unless there is significant supplementary income.
Cantons where the process is automatic
Several cantons calculate the reduction based on your most recent tax assessment and send you a decision without any application on your part:
- Vaud: decision sent each autumn.
- Geneva: decision sent each year to all potentially eligible taxpayers.
- Neuchâtel, Fribourg, Jura: partially automatic process depending on the case.
- Valais, Bern: process partially automatic for social assistance recipients.

Cantons where you must apply
In most German-speaking cantons and several French-speaking cantons, you must submit an active application every year:
- Form to be requested from the competent cantonal authority (often the social insurance office or the cantonal compensation office).

Documents to prepare
- Most recent tax assessment or current tax return.
- Salary certificates for the last 3 months (for working people).
- Pension statements (AHV, BVG/LPP) for retirees.
- Family allowance certificate if applicable.
- Household composition: spouse, dependent children, partner, students.
- Current health insurance policy.
- Bank account details for payment.
When and how the subsidy is paid
- The subsidy is generally paid directly to your health insurer (not to you), which reduces your monthly premium accordingly.
- In some cases, it is paid directly to your bank account (as a reimbursement).
- The decision is annual and must be renewed each year (except in automatic cantons).
Common mistakes
- Thinking it is reserved for social assistance recipients. The subsidy covers modest and middle incomes, not only social assistance recipients.
- Not applying out of pride. It is a social right funded by taxes, not charity.
- Forgetting to renew each year. Except in automatic cantons, the entitlement is not renewed automatically.
- Applying after the deadline. In some cantons, a late application means losing the reduction for the year.
- Overlooking changes in circumstances. Birth, job loss, separation: any change may create a new entitlement. Report it without delay.
